On sait moins qu'avec sa première collaboratrice, Jacqueline De Decker, elle a constitué une active association de trois mille coopérateurs malades et souffrants. À chacun d'entre eux, elle demande de parrainer par sa prière et son amitié un ou une missionnaire. Un réseau de correspondance s'est établi entre Mère Teresa et ces coopérateurs spirituels dont beaucoup sont alités ou séjournent depuis des années dans hôpitaux et maisons de santé.
